Job summary
This is an exciting time to join us as our newly formed Medicine and Community Care Delivery Group (CDG) brings together secondary care and community services. As we redesign pathways to improve continuity and outcomes, we are looking for a dynamic and experienced Nurse with an interest in heart failure to help bridge the gap between inpatient care, our virtual ward, and community heart failure services across Northeast Essex (NEE).
Working as part of an integrated multidisciplinary team, you will play a key role in supporting patients with heart failure to receive the right care, in the right place, at the right time, enabling better long-term condition management and reducing avoidable hospital admissions.
This role includes inpatient, outpatient, and home-based care and offers the opportunity to shape a responsive, person-centred heart failure service.
Main duties of the job
- Act as a clinical specialist in heart failure management, delivering holistic assessment, care planning and review across a variety of settings.
- Carry out single assessments and contribute to integrated care plans, supporting timely discharge and effective transitions across services.
- Develop and implement therapeutic care plans and programmes, delegating appropriately while providing clinical supervision to junior staff and support workers.
- Provide patient-centred education and self-management support to patients and carers, promoting independence and activation.
- Support optimisation and titration of heart failure therapies in line with NICE guidance and in collaboration with cardiology and primary care teams.
- Use clinical reasoning to monitor and evaluate progress and adjust care plans accordingly.
- Act as a care coordinator where required and participate in multidisciplinary case reviews and ward rounds.
- Supervise and support the professional development of more junior nurses, support staff, students and volunteers.
- Lead or contribute to clinical audits, research and service evaluation within the heart failure pathway.
