Job summary

An exciting opportunity has arisen for a fixed term Senior Community Physiotherapistto join the Northeast Integrated Neighbourhood Team. The role is based in Leiston. A full driving licence and access to a car for work are essential.

Community experience is preferred but not essential, as full training will be provided. Our team is motivated and forward-thinking, consisting of Therapists, Nurses, Social Workers, and Support Staff. We recently merged our Health and Social Care teams to form an Integrated Team, offering a new model of care and innovative ways of working.

The primary focus of the therapy team is to provide short-term rehabilitation in the community, supporting timely hospital discharge and preventing unnecessary admissions. This is achieved through tailored exercise programmes, equipment provision, and functional interventions that promote independence. We visit patients in their own homes, care homes, and nursing homes, as well as offering rehabilitation within the gym at Aldeburgh Community Hospital. We also run clinics, such as GLINT (Geriatric clinic) and community OP clinic.

Our team works closely with local GP practices and benefits from a wealth of experience and knowledge among its members.

Main duties of the job

  • To effectively triage and schedule all therapy referrals to the service in accordance with Trust's policies and procedures and advise junior colleagues on more complex issues.
  • To manage a caseload of patients with a range of multiple and complex health needs.
  • To utilise a high level of clinical knowledge and experience to carry out assessments, setting objectives and implementing appropriate treatment, as part of the holistic care plan for individuals and/or groups.
  • To formulate an individual management/treatment plan for each patient using wide range of advanced assessment, clinical reasoning and treatment skills, including health coaching and promotion of self-management skills.
  • To ensure interventions are evaluated and plans modified to ensure effectiveness and timely discharge for the whole therapy team.
  • To provide training and advice to patients and carers to facilitate their care plan in complex situations.
  • To adhere to agreed national, professional and Trust standards.
