Offers are open for an enthusiastic and passionate candidate to join the department of Older People's Medicine at Ipswich Hospital, part of East Suffolk and North East Essex foundation trust as a substantive consultant. Full time, part time and flexible working opportunities will be considered. It is an exciting time to join the trust as we plan the roll out of Epic - our new electronic health record in 2025.

We care for patients in their home using the virtual ward, and through the whole span of their journey within the acute hospital - from specialist assessment in the emergency department to seamless specialist care in our frailty assessment base. A truly patient-centred care model which is underpinned by expertise in specialist geriatric assessment.

Main duties of the job

The successful candidate will run ward rounds on our virtual ward supported by a Staff Grade doctor or SpR. The post holder may act as the clinical supervisor for the junior team. This will involve a daily consultant led board round on an online platform (TEAMS). The post holder will be expected to provide appropriate clinical leadership to the progressive, innovative multidisciplinary ward team.

The post holder will have scheduled time working in the Emergency Department as part of the FIT team, completing frailty assessments of patients at the 'front door'. These patients can then be seen in FAB and the successful candidate will be required to cover 2 sessions per week whilst listed on the time table below the actual session will vary from week to week as cross cover is included in the PA calculation. This role would support REACT services with clinical expertise they link in with the community Dementia Intensive Support Team, social services and the Red Cross to support people in through a short term deterioration in health and function. During the time in FAB/ front door frailty you may also hold the "hot phone" this is a mobile phone which enable any GP or community therapist to phone for advice or to book a same/next day patient into FAB. Our FIT team also use this for support and some appointments each day are reserved for direct transfers from ED for patients who can probably be discharged but need a more comprehensive/specialist assessment before leaving.

This clinic is established in some localities and being developed in others.

We invite those candidates with sub-specialist skills and attributes, for example, onco-geriatrics, orthopaedic liaison, or virtual ward community experience, and we would be happy, where possible to negotiate time within the timetable to allow these to flourish.
