Job summary

An exciting opportunity has arisen on Langham Ward following a successful quality improvement project to improve flow and patients journey on the medical wards.

We are seeking enthusiastic individuals who have a passion for improving the patient experience and supporting the patient's journey from admission to discharge.

Main duties of the job

You will need excellent communication skills as you will be liaising with members of the multi-disciplinary team, discharge team, patients and their families/carers, as well as a sound clinical knowledge. The successful applicant will be required to work as part of the MDT in planning of patients care whilst also considering early discharge plans. You will be required to be confident at managing difficult conversations, challenging decisions and plans whilst also adapting your communication style according to the situation, which will be varied. Capturing qualitative and quantitative data surrounding the discharge process will also be essential.
