Job summary

We have an opportunity for experienced and forward-thinking Occupational Therapists / Physiotherapists to join our award winning Reactive Emergency Assessment Community Team (REACT) .

The dynamic team is based in the community and in-reaches into the Accident and Emergency Department (A&E) at Ipswich Hospital, and the Frailty Assessment Base (FAB), also based at Ipswich Hospital. You will be working with patients in A&E, FAB clinics or in their own homes, focusing on admission avoidance and preventing the conveyance of ambulances to hospital. You will use a reablement approach to help them maximise the individual's independence and make informed choices about their current and longer-term care needs. You will work with a wide variety of internal and external services. We are looking for talented individuals who will be confident, flexible and professional, with the clinical skills required to manage a diverse, complex caseload within a dynamic environment.

You must hold a valid UK driving licence and be able to travel to patients' homes using either their own car or one of the team's pool cars. Both teams work across Ipswich and East Suffolk, with a central base in Ipswich, with proposed plans for further bases in Woodbridge and Stowmarket.

Main duties of the job

To plan and organise a designated caseload of complex patients within the designated service and own professional activities under the management of Team Leader.

To work with the patient and carers to identify and meet therapy goals, using standardised and therapy assessment tools. Using professional and theoretical knowledge, and specialist clinical reasoning skills to monitor, evaluate and modify treatment interventions to measure progress and ensure effectiveness, involving patient, carer and MDT as appropriate.

To apply an enhanced understanding of effects and impact of illness, frailty and disability on patient's abilities and provide advice and education regarding lifestyle changes / adaptations to their physical and social environment.

To use specialist technical, physical and mental skills in the provision of patient interventions.
