Job summary
'One area that makes me proud is our determination to improve patient care. We have a time garden which provides a tranquil outdoor space for patients and families. We also have end of life blankets, each hand knitted, one to be used per patient that softens the furnishing for our patients and families. These blankets can be taken home by families or are donated after use to dog rescue centres' Dr Hattie Roebuck, Consultant in Palliative Medicine and Guardian of Safe Working.
The job plan is expected to consist of 10 Programmed Activities (PAs) at Colchester Hospital.
Exact details of job plan will be agreed with the successful candidate after their appointment.
A 1-in-5 second on-call service is worked by the hospital and hospice consultants, managing hospital, hospice and community palliative care patients, predominantly by telephone support. This is compensated as per national contractual arrangements, by at least a 3% supplement. Any agreed or emergency additional work is compensated via time off in lieu or payment
The post-holder will:
- Participate in identifying areas for development of services and, where appropriate, the consequences of implementation
- Maintain and provide statistical information as required
- Be aware of and participate in the formal and informal staff support systems available within the Hospital
- Undertake other roles as may be required in maintaining and furthering the work of the Hospital where appropriate
Main duties of the job
The key duties and responsibilities are:
- Review those hospital inpatients referred to the specialist palliative care team who require consultant level assessment.
- Ensure patient care is evidence based and/or best common practice
- Ensure clinical practice complements and facilitates the provision of multi-disciplinary care, recognising the physical, psychosocial, emotional and spiritual needs of the patient and family
- Ensure that all prescribing is in accordance with the hospital policy and statutory requirements
- Ensure accurate and timely clinical information and records are maintained on all inpatients
- Liaise with primary and secondary care as appropriate to ensure continuity of medical care and at times be available to advise and support in the management of palliative patients transferred to the community setting
- Conduct outpatient assessments in clinic as required. The post is based at Colchester Hospital. In exceptional circumstances, the consultants might be asked to support their colleagues at Ipswich Hospital and vice versa.

The department works to build strong relationships with every specialty at the Trust, working collaboratively to ensure high quality palliative care is delivered as needed to patients, irrespective of their diagnosis.
We have been a pro-active & integrated part of the hospital's response to the Covid-19 pandemic. We are proud that in January 2021 an 80% increase was recorded in the number of patients reviewed by specialist palliative care. We aimed to deliver and advise upon the symptom management of these patients, whilst also supporting our colleagues emotionally.
- On average, each month the team reviews 180 different patients, providing 550 patient contacts. 92% referrals are seen within 24hrs.
- The proportion of cancer to non-cancer cases is 50:50.
- A 7 day a week, 9am-5pm, face to face service is delivered by the nursing team alongside a Consultant on call 24/7
- Within the team we have 3 CNS nurse prescribers and are training a further CNS to fulfil this role
