Job summary

Due to retirement, The Diabetes and Endocrine Centre and Diabetic Foot Team based at Ipswich Hospital have an exciting opportunity for a Diabetes Specialist Nurse with a specialist interest in the care of the diabetic foot.

You will be based at the Ipswich Hospital splitting your time between the Diabetes and Endocrine Centre and the Diabetic Foot Clinic working as part of this multidisciplinary team, participating in the delivery of integrated diabetes care for people with diabetes across East Suffolk

This post will be mainly based in the outpatient setting with close working relationships with our ward based and community based nursing team colleagues promoting diabetic foot education, optimising glycaemic control and running autonomous specialist clinics. We offer secondary care services to patients with both Type 1 and complex Type 2 diabetes as well as covering specialist areas of diabetes in pregnancy, diabetes related renal disease and also have a substantial amount of patients using technology and insulin pump therapy which you will be expected to support.

The team is dynamic and forward thinking and have expanded to provide an integrated diabetes service for East Suffolk. The team has won several awards including the Quality in Diabetes Care award on 2 occasions and the HSJ award.

Please note, this is a fixed term/secondment position for 23 months. Internal candidates will be a offered a secondment only.

Main duties of the job

  • To act as a recognised clinical expert in all areas of diabetes care and management
  • To provide expert clinical practice in the management of the most highly complex caseload of patients registered with the Ipswich Hospital Diabetes Foot Clinic with the aim of maintaining mobility, viability, independence and relieving pain.
  • To work as an autonomous practitioner within the wider multidisciplinary team, providing a continuous service for patients diagnosed with or at risk of diabetes foot disease, providing holistic care, advice and support in both in-patient and out-patient settings.
  • To provide expert on going education and support for adult patients and their carers with regard to all areas of diabetes care, foot care and management trust wide
  • To act as expert resource and Foot Champion for health care professionals in both primary and secondary care settings, offering specialist advice and support regarding all aspects of diabetes and its associated therapies and complications. Play an effective role in the prevention of foot ulcers and lower limb amputations by educational interventions and screening high risk people
  • To act as mentor and role model to all band 6 Diabetes Specialist Nurses, providing expert knowledge and advice, clinical supervision and clinical governance to ensure patient safety.
