Job summary

As part of a major investment programme, at Ipswich Hospital, agreement has been given for a significant investment to deliver a truly transformative model for management of inpatient medical and older persons care. This model has been designed collaboratively by our consultant leads across acute medicine, older people's services and medical specialties. It aims to deliver:-

  • Appropriately resourced 7-day consultant working
  • Strengthened GIM rota, improving rotas and providing a better balance of workload across each day of the week; reducing the burden on clinicians associated with Medical Take by reducing the frequency and bolstering the post-take support
  • Innovation in patient care and service delivery, allowing the right patient, to see the right clinician, at the right time (7 days per week), ensuring patients are managed from the outset, on the most appropriate ward for their condition(s).
  • Early access to specialist advice
  • Overall, to deliver greatly improved quality of care for our patients whilst supporting a long-term, sustainable set of consultant job plans

We are looking for a full time Consultant in Diabetes & Endocrinology to support this ambitious new medical inpatient model who wish to embrace the technological advances recently made in patient care and who will contribute to the evolving vision for the future of our multi-specialty medical team.

An Enhanced Relocation Package including a Lump Sum Payment may be available for the right candidate.

Main duties of the job

The successful applicant will have a shared responsibility for the inpatient bed base (consultant of the week - CoW) currently 2 weeks out of every 8 tough this will change on the appointment of an additional consultant. This appointment will facilitate weekend specialist working and allow a reduced commitment to the GIM rota with the aim of withdrawing from this in due course. When providing CoW services, clinic work does not occur, to facilitate provision of dedicated inpatient care. The successful applicant will be expected to join the general medical on call rota. This is currently approx. 1 in 16, weekends only general medical take, including post take ward round duties. This is 1 weekend day approximately every other month and provides 12 hours within the hospital, then on call over night from home. This rota id under review

Each consultant provides a general diabetes clinic, at least one specialist diabetes clinic (renal, pump, antenatal, foot) and one or more endocrine clinics per week when not on COW duties. Some consultants support the transitional clinic. There will be opportunity to develop in one or more specialist diabetes areas whilst undertaking general clinics.

Job description

Job responsibilities

The post holder will be expected to provide appropriate clinical leadership to the progressive, innovative multidisciplinary team, in conjunction with the other consultant and ward sisters and senior diabetes nurses. The post-holder will help develop the diabetes and endocrinology services appropriate to the need of the population, in partnership with the other consultant colleagues in the department and with support from Clinical Director.

The post holder will be the clinical supervisor of some of the juniors (shared), and there is the opportunity to become an educational supervisor after appropriate training if desired. This activity is funded separately by the Post Graduate Centre. There is an established faculty of supervisors to provide support and education. We offer training to Cambridge and University of East Anglia medical students and the post holder will be expected to help deliver Module 6 education for UEA medical students as part of a rostered programme.

We have an active post graduate centre and the post holder will be expected to contribute to the foundation, CMT, VTS and SpR teaching programmes in addition to student, nursing and allied health professionals as required.

The departmental service and educational meeting is every Monday lunchtime and each consultant takes turns to deliver this. There are grand rounds, Schwartz Rounds and a wide variety of other educational opportunities.

All consultants in the department have a continuing and individual responsibility for the care of patients in their charge and a general responsibility to provide, in conjunction with their colleagues, an effective Service to meet the needs of the local population, within the resources available.

The post-holder will:

  • Work with colleagues to provide a consultant-led service with the highest standard of care
  • Work with the multi-professional team to develop care pathways and clinical guidelines
  • Develop and maintain good working practices within wards, outpatients and A&E
  • Collaborate and promote close working links with colleagues in tertiary centres, community services and primary care
  • Offer support and leadership to junior medical colleagues, nursing colleagues and other members of the multi-disciplinary team

The proposed timetable is an example of the work required but is not rigid and will be very flexible depending on service needs and interests of the applicant. For instance the timetable indicates the foot clinic but this could be replaced by another specialist clinic by negotiation. The Trust provides 1.5 SPA for all consultants, extra SPA sessions are offered for specific roles and will be negotiated as part of job planning.

A formal job plan will be agreed between the appointee and CDG lead on behalf of the Divisional Medical Director three months after the commencement date of the appointment.

The job plan will then be reviewed annually. It will cover all aspects of a consultants professional practice including clinical work, teaching, research, education and managerial responsibilities. In addition, it will include personal objectives, details of their link to wider service objectives and details of the support required by the consultant to fulfil the job plan and the objectives.
