An exciting opportunity has arisen to join the Trust as a Patient Safety Manager. The Trust are adopters of the Patient Safety Incident Response Framework, providing an opportunity to respond to patient safety incidents in an effective and proactive manner, ensuring the principles of just culture are embedded.
The post holder has a key role in the management of the Trust's Patient Safety Incident Response Plan. You will be part of the team that leads investigations across the Trust in accordance with the Trust's Patient Safety Incident Response Plan. The successful applicant will show outstanding communication skills and demonstrate the ability to work collaboratively with a wide range of staff and specialties. Working as part of a team, you will work with the clinical teams to identify quality improvements through investigation of incidents and identification of emerging risks through incident reporting.
Main duties of the job
- To strengthen and promote a culture of patient safety, providing a visible presence to front line staff to support the investigation of, and learning arising from incidents to ensure change in practice takes place.
- A key role within this will be to identify, develop and deliver training in incident reporting and investigation techniques to include root cause analysis skills, and action planning, to ensure that lessons learned from incidents are robust and ensure that all staff are empowered to take action to minimise risk to patient safety, patient experience and clinical effectiveness.
- The post holder will carry an individual portfolio of work, which is discrete, measurable and which drives the direction of travel in relation to patient safety. This will include leading on specific patient safety projects/initiatives as directed by the Chief Nurse, Chief Medical Officer, or the Associate Director of Clinical Governance.
- To ensure the provision of a robust incident reporting system and ensure that relevant and regular training programmes are provided to support staff in the reporting of incidents
- To action and administer significant incidents promptly and agree the method of investigation, on behalf of the Associate Director of Clinical Governance. The Patient Safety Managers are expected to provide critical challenge where required to support the Executive Lead.

- As patient safety ambassadors to formally support the Trusts patient safety/clinical governance agenda to ensure the achievement of objectives and relevant assurances to the Board and external agencies including compliance with external alerts.
- To leading on specific patient safety projects as directed by the Chief Nurse, Chief Medical Officer and/or the Associate Director of Clinical Governance, developing and maintaining a portfolio of work providing a visible presence in the clinical areas.
- To develop, support and embed patient safety strategies across the Trust to minimise risk to patient safety, patient experience and clinical effectiveness.
- To actively promote opportunities to learn and enhance patient safety and deliver education and training for all groups of staff
- Provide advice and support across the whole Trust and enable staff to follow the patient safety and risk management policies through a consistent system which is implemented and embedded in practice across the Trust
- To attend the clinical areas in response to patient safety incidents of concerns for fact finding, identification of immediate actions to maintain patient safety and confirm the level of investigation required.
- Act as lead investigating officer for all incidents identified as requiring investigation as a patient safety incident investigation (PSII). Ensure that the actions developed throughout the investigation are allocated to the appropriate individual or group and that the Divisional Governance team are updated by adding to Datix and notification.
- To quality assure evidence being supplied that complete action plans, further attending wards and departments to provide further assurance of action following incident occurs.
- To provide advice, guidance and support on existing and new legislation in the event of incidents to support clinical and managerial staff in managing the incident. To use clinical expertise in providing specialist support and advice in areas of clinical practice related to patient safety, seeking senior support where required.
- To support the clinical divisions to develop robust streams of work to support the developing patient safety strategy across the organisation; in particular in relation to the use and development of improvement tools and techniques which allow for the critical analysis of situations and experiences to reduce harm.
- To act as Family Liaison Officer (FLO) for those incidents requiring investigation as a PSII.
- To provide statistics, analysis and reports to the relevant divisional governance groups to identify trends, hotspots and areas of incidents and significant risk that need to be brought to their attention, providing comparative data for benchmarking.
- To support the Associate Director of Clinical Governance in the preparation and presentation of regular reports for the Director of Nursing to present at the relevant Trust committees or patient safety groups, outlining comparative data trends, to consider potential hot spots and suggest actions to mitigate risk.
- To compile summaries of patient safety recommendations from the investigation of incidents, facilitate the sharing of lessons across the divisions and monitor action plans to ensure implementation within the agreed timescales.
- To develop and maintain good communication links to support cross-organisational reporting, investigation and learning of incidents between the Trust and other Trusts throughout the region.
- To contribute to the combined Annual Quality Report.
