Do you have a passion for Stroke rehabilitation and want the opportunity to work with a dynamic team to provide rehabilitation to patients out of hospital? The Community Stroke service is seeking a part time Occupational Therapist to work within our team.
You should have knowledge in basic assessment, neuro rehabilitation, goal setting, care planning and good communication skills. Senior support will be given to develop this further and improve skills and confidence
This post is community based and therefore it is essential the post holder is a car driver.
Experience in Health coaching, Bridges or a similar model would be advantageous, otherwise training would be provided for the successful candidate.
Main duties of the job
To work as part of the integrated team to provide seven day nursing or therapeutic input to patients with the aim of returning them to maximum independent living, within a stimulating environment actively promoting self-management on all activities of daily living.
To provide assessment, differential diagnosis, advice, training appropriate intervention and evaluation of planned care.
Work with colleagues across health, social care, education, mental health and voluntary sector to ensure holistic and joined up delivery of care.
Empower patients to make decisions about their own care and encourage them to exercise their rights and choices, signposting them to relevant community and voluntary services and supporting patients carers and assistants with management of risk.
