Frailty Nurse

Band: 7, £46,148 - £52,809 Per annum, pro rata if part time
Hours: Fixed Term, Full time, 40 per week, all MKUH roles will be considered for flexible working
Exciting opportunities within Acute Assessment and Frailty Team (AAFT) have arisen! After having successfully recruited TWO new Band 6 Clinical Nurse Specialists, we are now recruiting for TWO Band 3 Frailty Healthcare Assistants (HCA) and a 15-month fixed-term Band 7 Frailty Nurse. Working alongside Nursing and Therapy staff, you will be working in multiple environments, such as SDEC, ED and wards, to promote robust and timely patient discharge.
For the role of a Frailty nurse, we are looking for dynamic, confident individuals with medical, elderly care, frailty or SDEC experience to provide specialist frailty care, as part of a multidisciplinary team. Additionally, you will be responsible for leading the way in reducing the length of stay for patients within ED and SDEC, co-lead the auditing of the acute SDEC frailty pathway then presenting findings to Directorate.
Successful applicants will be a champion for change, being able to communicate effectively across the Trust maintaining confidentiality. You will show high levels of empathy and a passion for providing dignified and holistic care whilst upholding the NMC Code of Conduct.

Main duties of the job
o To be responsible and accountable for the delivery of high quality, innovative clinical practice within SDEC/front door services.o Proactive clinical decision making underpinned by an advanced level of theoretical and practical knowledge and be able to demonstrate improved patient outcomes.o Coordinate and lead a Trust-wide evidence-based frailty pathway in conjunction with the Consultant Geriatricians, fellow Frailty nurses and Specialist Frailty Therapists.o Act as a Highly Specialist Frailty Nurse resource for the Acute Assessment and Frailty Team leading MDT education and training in acute frailty care alongside the Consultant Geriatricians, Frailty Nurses and Specialist Frailty Therapists.o Develop, implement and lead integrated working practices with Community partners to facilitate timely and effective discharge and admission avoidance for patients with frailty at MKUH, in conjunction with Consultant Geriatricians and Specialist Frailty Therapists.o Work collaboratively with other Lead Nurses e.g. Dementia Lead and Falls Prevention Coordinator to drive Trust - wide improvements in the care of acutely ill patients with frailty at MKUH.o To work autonomously using history taking and examination skills within the multidisciplinary team, guidelines and protocols.
