Job summary
The Colorectal Clinical Nurse Specialists work in collaboration with the Colorectal Multidisciplinary Team, providing clinical expertise, assessment of care and health needs of patients, with colorectal cancer conditions, and their families. To assist with the efficient day to day operational management and service delivery of the Colorectal Oncology Service in partnership with the Consultant Teams and Multidisciplinary staff, ensuring efficient and effective use of resources to optimise quality and national targets. They will network with Colorectal Cancer colleagues at local and regional meetings, to ensure continuous learning, sustaining best practice and service innovation. They will be an expert practitioner within the speciality and have the ability to manage complex clinical situations, providing guidance, supervision and or training appropriate to peers and junior members of staff. Acts as key worker for a clinical caseload of patients receiving treatment for a colorectal cancer, working to co-ordinate care and provide easy access for on-going support for patients. He/she will be actively involved in monitoring and developing Colorectal Oncology Services to ensure patients receive sustainable high quality care that is safe and effective. Support the referral process team aiming to deliver waiting times in line with the standards set out in The National Cancer Targets and Trust Policy.
Main duties of the job
Clinical
Puts quality at the heart of practice by delivering evidence based individualised and personalised care through holistic needs assessment, planning and evaluation of care and all care interventions.
Acts as key worker for a clinical caseload of patients receiving treatment for a colorectal cancer, working to co-ordinate care and provide easy access for on-going support for patients.
Assesses people's information needs and provides relevant information to meet those needs.
Enables people to access, identify, appraise and interpret information to determine their own future.
Provide clinical expertise, specialist advice and support across service boundaries.
In conjunction with nursing colleagues, contribute to nursing strategy for the diagnostic, surgical, medical, clinical and follow-up Colorectal Cancer Service development, ensuring the pursuit of excellence in care.
Defines a clear care patient pathway, refers to other professionals and sign-posts to other agencies as appropriate.
Promote the service and clinical excellence by publishing and presenting innovations, audit and research locally and nationally.
Influences the development of policies and clinical guidelines within the Colorectal Cancer Speciality.
To independently run nurse-led clinics to manage the follow-up pathways of; colorectal cancers.
