Job summary

This is a great opportunity to apply for a Consultant post in Geriatric Medicine, working as part of the consultant team as we expand our Hospital at Home service. Our Hospital at Home team is a leading service, well established and currently managing a case load of approximately 40 patients. Patients with acute illness are treated in their own homes with Tier 4 interventions, provided by a multidisciplinary team led by consultant geriatricians. The majority are frail patients but we also serve less frail patients with a wide variety of acute conditions.

This post has been created to support our rapid expansion of service, which brings exciting opportunities to manage more inpatients in their own homes, as well as to liaise with community teams to escalate treatment in admission avoidance.

Further specialist interests work is open to negotiation depending on the expertise of the applicant. This post is based within the Older Persons' Unit at the Royal United Hospital (RUH) Bath, with a main focus on Hospital at Home work, though we are interested to hear from applicants who would prefer to work mainly in our in hospital team - we have consultants ready to move into Hospital at Home if inpatient work is reallocated.

Main duties of the job

We have 5 wards for older people, 2 trauma wards, 16 consultants, 5 SpRs, numerous trainees and 14 medical nurse practitioners. Several consultants have research interests, and we all teach undergraduates from Bristol Medical School. We encourage involvement in the development of services or other aspects of geriatric medicine. There is a separate Geriatric Medical take and the post holders will take a share in that on weekdays; consultants also work 1 in 6 weekends (frequency set to reduce with appointments) with 2 Consultants working (one full day seeing admissions, one half day reviewing ward patients with our nurse practitioners). There is no on call responsibility, though some geriatricians choose to join the GiM on call rota additionally.

Clinical Experience

Essential

  • Clinical training and experience equivalent to that required for gaining UK CCST in Geriatric Medicine

  • Experience of Rehabilitation in a variety of settings
  • Ability to take full and independent responsibility for clinical care of patients
  • Demonstrable experience in discharge planning and frailty

Desirable
