Job summary
The Family Liaison Facilitator (FLF) will be part of the Family Liaison team in Medicine and surgery. Their main role is to ensure that relatives are kept updated on their loved ones during their stay in hospital. Members of the team will provide an essential link between patients and their next of kin and provide a professional, compassionate and supportive service to patients, their families and carers. The role is key in the support of the emotional well-being of patients and their families/carers.
Provision of this service will improve patient/family/carer experience and well-being by maintaining a line of communication during their time in hospital thereby enabling ward based clinical staff to focus on direct patient care.
The team will support wards within the medical and surgical division.
The service will operate from 8am to 8pm 5 days a week, with an occasional need to work a weekend or a bank holiday.

Main duties of the job
- The post holder will work closely with the shift co-ordinators on the above wards to identify the relatives of patients who are on the ward and need contacting. The Family Liaison Facilitator will contact relatives using a Trust mobile phone and log the calls for sharing with the wards.
- The post holder will be supported by the PSCT and given 'scripts' to make the calls. Clinical information will not be shared other than generic information about the ward - doctors rounds, mealtimes, drugs rounds, etc. The purpose of the call is to enable 2-way communication as well from the relative to the patient/clinical staff to ensure that care provided is patient-centred - e.g. how best to communicate with the patient, any information that will help support their care, etc.
- The post holder will attend the 'white board' rounds.
- Where there are changes to visiting on wards, due to infection control issues, the post holder will work with the ward manager to inform relatives of any changes and advise about the virtual visiting and keeping in touch services.
- The post holder will keep in daily communication with the PSCT and switchboard and respond to any relative enquiries for the above wards
