A sought after and exciting opportunity has arisen to join our Neonatal Community Outreach (NCO) team during inspiring times of transformation. We have a long established NCO team with over 23 years experience and strive to further enhance the care we provide for families. We are looking for a motivated and enthusiastic neonatal nurse, who is passionate about quality improvement and improving the experience of babies and their families.

This role, under the support of Band 7 NCO lead, is responsible for delivering clinical care to preterm infants and their families during and following discharge from hospital. You will provide direct support for families under a family integrated care model ensuring a seamless transition to home whist being advocate for the family. You will communicate with multi-disciplinary teams including babies' named consultant to implement and review individualised care plans.

Main duties of the job

You will actively support the Neonatal Community Outreach Lead, assisting in the safe, effective, and efficient management of the Outreach team caseload. You will be able to participate in the assessment, planning, implementation and evaluation of babies and their families following discharge from the Neonatal Unit at The Dyson Centre for Neonatal Care, RUH Bath, who meet the referral criteria to the service.

You will be part of an MDT working from a level 2 Local Neonatal Unit within the Southwest Neonatal Network. The unit has 21 cots comprising of 7 intensive care/ high dependency cots, and 14 special care/Transitional care cots.

  • Responsible for the delivery of effective quality care to preterm infants in the community setting, with the support of the Band 7 Neonatal Community outreach lead. Will require own car and current UK Driving Licence.
  • Contribute to the discharge process for infants
  • Act as a resource of specialist knowledge and expertise to ensure optimum and appropriate care delivery to babies and their families in the community
  • Demonstrate a knowledge of clinical nursing practice by delivering evidenced based care
  • Contribute to resource management in the provision of Outreach services
  • Support the implementation of change in response to clinical, organisational or service requirements
  • Promote a positive and professional working environment
  • Identify and refer appropriately, infants requiring Safeguarding assessment
