Children's Respiratory Nurse

Job summary

You will work as part of the Children's Community Nursing Team and Children's Nurse Specialist Team, supporting and managing Children within the community and in the hospital with diagnosed Respiratory condition.

To assess, plan, implement and evaluate care delivery that supports the child/young person to remain in the community setting and where possible prevent hospital admission. Where hospital admission is required, you will be able to support the inpatient ward team to meet the needs of the child or young person and support with timely discharge.

It is expected that you will work collaboratively across local and national networks for asthma, cystic fibrosis, long term oxygen and tracheostomy, ventilation at home management.

You will participate in service development and collaborative working across health, social care and multi-agencies for patients diagnosed with complex respiratory conditions.

Main duties of the job

To work autonomously, managing own caseload of patients to assess, plan, implement and evaluate care.

To provide specialist advice and support to children and young people (CYP) and their families to improve self-management of their condition within the community setting, in hospital and in nurse led clinic's.

To appropriately triage, visit and support patients who may be unwell.

To provide support that is appropriately planned to meet the individual's needs, empowering them to manage their care, to recognise deterioration/acute episodes requiring immediate specialist attention and how to access the necessary support.

Ensure nursing practice is in accordance with the NMC/HCPC Code of Professional Conduct and any specific competencies for the role are completed.
