We are looking for a compassionate and motivated Health Visitor to join our 0-19 Service team. You will work with children, families and communities to promote good health, prevent illness, and reduce health inequalities. You will lead the Healthy Child Programme, provide support through home visits, and work with a variety of professionals to ensure the best care.

Your responsibilities will include assessing and planning care, safeguarding children, collaborating with multi-agency teams, and ensuring standards of practice. You will also be involved in staff development, case management, and contributing to service improvements.

Main duties of the job

Reporting into the Team Leader, your responsibilities will include:

  • Lead the Healthy Child Programme and provide evidence-based advice and support to families through home visits and other settings.

  • Conduct continuous health assessments of clients/families, planning and implementing public health interventions based on identified needs.

  • Collaborate with multi-agency teams to deliver holistic planning of care, including referrals for families in need of urgent support.

  • Participate in health promotion programmes, including immunisation and breastfeeding initiatives.

  • Deliver universal plus/partnership plus services to vulnerable groups

  • Conduct risk assessments and take management action to avoid or reduce risks.

  • Prescribe medications using the Nurse Prescribing Formulary, when appropriate.

  • Attend safeguarding meetings and follow safeguarding guidelines
