Job summary

We are seeking a motivated, dynamic and enthusiastic Community Health Support worker to join the 0-19 SAIS Team ( Sheffield Adolescent Immunisation Team).

The 0-19 Service in Sheffield is committed to delivering high quality care to children 0-19yrs and their families. Our delivery model for Children and Young People's Service is based on Early Intervention, prevention and the promotion of Public Health.

This is an opportunity for individuals who are committed to partnership working and who are able to demonstrate a willingness to adapt to new ways of working to effectively deliver the Vaccination & Immunisation/Healthy Child Programme.

Applications are welcome from experienced and New Health Support workers with an NVQ in either relevant health or social care study at level 3 or evidence of an equivalent level of knowledge and skills and experience.

Main duties of the job

To work with and under the direction of the Specialist community public health Nurses(s)and the Community Public Health Staff nurse(s), reporting effectively on all aspects ofcare relating to children, families and carers.

To assist the 0-19 service in the delivery of health promotion activities in accordance withthe healthy child programme and the adolescent immunisation programme.

Under the supervision of the Specialist community public health Nurses(s) and theCommunity Public Health Staff nurse(s), support the delivery of national and local publichealth campaigns, including: healthy weight and nutrition, Immunisation and vaccination,Sexual health, health lifestyle, maximising learning and achieving, and transition.

To support the 0-19 service and the adolescent immunisation programme in conjunctionwith the 0-19 Admin team and child health department in the processing of parentalconsents and contra indication of the Vaccination programme - including the Parentaldiscussion of vaccination and immunisation under the supervision of the Specialistcommunity public health Nurses(s) and the Community Public Health Staff nurse(s)Booking and arranging venues, and supporting the delivery of the sessions
