Job summary

  • To work as a Community Nursery Nurse within the 0-19 Service, the post holder is accountable to, and supervised by the 0-19 Team in delivering the Healthy Child Programme to children and families aged 0-5 years.
  • Support the delivery of the Sheffield 0-19 Service specification.
  • To work in a variety of settings, including child health clinics, Family Hubs, community centres, GP surgeries, schools, and clients' homes.
  • Excellent communication skills are required for this role.
  • Candidates are required to have a good knowledge of child development and an understanding of safeguarding.

Main duties of the job

  • Delivery of the Healthy Child Programme under the supervision of the 0-19 Team.
  • Support the 0-19 Team in the delivery of clinics, packages of care, public health interventions and the day to day functioning of the team.
  • To plan and manage workload and be able to work independently.

Job responsibilities

Clinical:

  • To deliver elements of the Healthy Child Programme and 0-19 Sheffield Service Specification under the supervision of the 0-19 Team.
  • Use evidence-based tools to inform assessment of children.
  • To work with families in a variety of settings including home visits, providing evidence-based advice and support.
  • To deliver an agreed programme of care by a continuous cycle of assessment, planning, implementation, and evaluation.
  • To liaise effectively and work together with other members of the multi-agency, multi-disciplinary and other teams to ensure cohesive planning of care for individuals, families, and communities.
  • Identify and prioritise those families who require a rapid response and refer to other agencies.
  • Promote and use effective communication and liaison with parents, carers and professionals, and other services.
  • Work according to safeguarding guidelines as defined by local and national policy to meet the safeguarding needs of children, young people, and families.
  • Maintain safe working conditions by monitoring and reporting areas of concern immediately to the Team Leader
  • Contribute to the identification of health needs of the community.
  • Assist the 0-19 Team in addressing individual, family, and community health needs.
  • To allow student SCPHNs, pre-registration nursing students, and new staff to shadow.
  • Contribute to the delivery of service Key Performance Indicators.

Professional Development

  • To work in accordance with relevant national and local policy and procedure
  • Engage in regular clinical supervision.
  • Deliver evidence-based care, maintaining own continual professional development.

Management/Staff Development

  • To work in accordance with the relevant Sheffield Childrens NHS Foundation Trust policies.
  • To participate in the orientation and induction of new staff.
  • Accurately record and collect data/statistics as required by the service.
  • To be responsible for the maintenance of day-to-day caseload management.
  • Daily prioritisation of workload
  • To support the team in the identification of local resources
  • Participate in regular and thorough assessment of risk, and effective management action to avoid or reduce risks to self and others.
