Haematology/Oncology Care Co-Ordinator (Band 3)

Job summary

The jobholder will work within the Haem/Onc CNSs as part of a multidisciplinary team, assisting the CNS's with actions arising from MDT outcome discussions

The job holder will be responsible for the coordination of the administrative function within the complex cancer pathways arranging investigations, appointments, and Multi-Disciplinary Team discussion in a timely manner to ensure waiting time targets are not breached and patient's needs are met.

Supporting patients and carers through the difficult journey they face. Ensuring patients have the appropriate support and extended services involved.

This position is not currently eligible for a certificate of sponsorship in line with Home Office guidance as it does not appear on the skilled worker visa occupation list.

Main duties of the job

  • Assist the CNSs to deliver patient centre, self-management support and education as necessary.

  • All adverse incidents and near misses should be reported through the Adverse Event Form (AEF) system and brought to the attention of the appropriate CNS.

  • Ensure all data related to patient care is entered onto the relevant computer systems, e.g. PPM, system one or PAS.

  • Take responsibility for the maintenance of accurate paper and electronic information ensuring confidentiality.

  • Discuss support options with the patient and their carer, with sensitivity, liaise with and keep CNS informed of outcomes

  • Telephone follow up patients as part of nurse led clinics after advice by CNS or Doctor. Escalating any problems appropriately
