Job summary

The post holder will work under the guidance of the Clinical Nurse Specialist. The role supports people who have been referred with a suspicion of or have a diagnosis of a gynaecological cancer. The aim of the role is to make sure that patients can access the right support, at the right time and to be the patient advocate. The Cancer Care Co-Ordinator will act as a keyworker, identifying a person's holistic needs, coordinating a person's care, navigating the health and social care system on their behalf.

Main duties of the job

The jobholder will assist in the overall implementation and evaluation of care as part of the multi professional team and provide a single point of contact for the patients/carers and will be expected to work to agreed protocols.

Main duties include:

  • Provide regular supported conversations with the person living with a suspicion of cancer and confirmed diagnosis, assessing their holistic needs
  • Answering non routine phone calls from clinically vulnerable patients and using knowledge, training, experience and resources to triage to appropriate intervention.
  • Completing post operative checks for non complex patients.
  • Utilise reports in the PPM database to monitor patients and identify delays for actions.
  • Makes contact with the person living with cancer to assess their wellbeing, provide basic telephone advice, and refer on or signpost .
