An exciting secondment opportunity has arisen for a AfC Band 7, full or part time position within the Corporate Patient Discharge Project. This project is a key organisational priority in line with our True North ambitions, and the role will be critical to supporting the project's successful delivery.

The post is for a joint Workstream Owner for the Criteria-Led Discharge Workstream, to drive the delivery of the workstream objectives:

To increase the % of patient discharges taking place before 12pm

To increase the % of patient discharges taking place at weekends

To reduce the % of patients in hospital with No Criteria to Reside (you may know this as MOFD or MFFD)

To increase the accuracy and completeness of discharge delay related data and Nursing Admissions data

The aim of the post is to develop and embed the practice of Criteria Led Discharge across the trust (excluding maternity and paediatrics). To be proactive, work collaboratively and be able to engage, teach and apply quality improvement principles to help change culture and practice across specialities and disciplines.

The successful candidate will work closely with the operational transformation team, clinical leaders and ward staff to support the successful delivery of the workstream milestones and objectives.

Main duties of the job

The aim of the post is to develop and embed the practice of Criteria Led Discharge across the trust (excluding maternity and paediatrics). To be proactive, work collaboratively and be able to engage, teach and apply quality improvement principles to help change culture and practice across specialities and disciplines:

  • To act as joint Workstream Owner within the Corporate Patient Discharge Project, with accountability and responsibility for delivering a Criteria-Led Discharge process across all in-scope wards and specialities.
  • To lead on criteria-led discharge training of predominantly Nurses/AHPs and collating and feeding back data to key stakeholders
  • To source and analyse criteria-led discharge and discharge delay data as required.
  • To produce a monthly workstream Highlight Report for the Project Board, and periodic presentation to the Project Board.
