This is an exciting role for a kind enthusiastic experienced nurse who is keen to join our cardiac nurse team at Harrogate District Hospital. We are looking for someone with cardiology experience both in hospital and community.
We are looking for someone who is keen in addition to Cardiac Rehabilitation to have an interest in supporting inpatients with heart failure and liaising closely with the community Heart Failure Specialist Nurses.
Main duties of the job
To deliver cardiac rehabilitation to patients from the Harrogate and Rural District in both the hospital and community as part of the integrated cardiac rehabilitation team
To provide support education for patients admitted with heart failure with complex health needs to Harrogate District Hospital.

To work independently and in conjunction with other members of the team to deliver this high-quality service to patients following cardiac surgery, ACS or PCI through face-to-face consultations and by telephone both in the hospital and in the community.
To provide education to inpatients with heart failure in Harrogate District Hospital and onward referral to the community heart failure nurse team.

- Evidence of study at degree level or equivalent
- Teaching or mentorship qualification or willingness to work towards

- Non-Medical Prescribing or willingness to work towards.
- Relevant cardiac courses
- Intermediate life support
Experience

- Minimum of 3 years post registration experience caring for patients with cardiac conditions in hospital or community.

- Experience teaching and presenting at meetings

- Knowledge of electronic patient records
