Position Summary:

As a Complex Discharge Specialist Nurse, you will take a lead role in ensuring safe, timely, and efficient discharge for patients who require complex assessments for on-going care in their own homes and care settings. Your focus will be on facilitating the discharge process to community services while aiming for discharge before 12 noon. Working autonomously, you will ensure patients are discharged at the earliest opportunity, promoting the use of family transport to reduce dependency on services.

You will be responsible for demonstrating expert knowledge of discharge options and services, providing support to patients, their families, and care workers in managing discharge plans, and addressing any concerns that may arise. Your leadership and collaboration at multidisciplinary team meetings will ensure patient needs and preferences are addressed, helping to achieve a timely discharge.

Main duties of the job

Key Responsibilities:

  • Identify Complex Discharges: Lead the identification of patients requiring complex discharge planning and facilitate assessments for community services, ensuring safe discharge from acute care.
  • Autonomous Discharge Planning: Ensure patients are discharged as early as possible, utilizing family transport where appropriate to reduce reliance on services.
  • Expert Knowledge & Support: Provide expert knowledge on available services, supporting patients, families, carers, and care workers in the discharge process, addressing any concerns and managing expectations.
  • Multidisciplinary Collaboration: Lead and contribute to multidisciplinary team meetings, advocating for patients' wishes and promoting advanced care planning to avoid unnecessary hospital admissions.
  • Support Ward Staff: Assist ward staff in developing their discharge planning skills and ensure documentation is in line with trust policy.
  • Involvement in Projects: Engage with internal projects focused on improving, updating, and revolutionizing how we approach patient discharge, contributing your insights and expertise to enhance practices.
  • Creative Problem Solving: Use your initiative to develop tailored, creative solutions to complex social challenges, ensuring patients are supported in their unique circumstances
