We are looking for an enthusiastic, motivated individual who has extensive knowledge of child development and experience of working with Children, Young people and families. The candidate must have excellent communication skills and an ability to work within a dynamic team of Nurses.
The job is to support Special School Nurses, Learning Disability Nurses and Community Nurses in providing a service for children and young people in special schools, primary care and the community that promotes their physical, social and emotional well-being.
This post is based in Northallerton in the Hambleton & Richmondshire locality but may occasionally be required to support the Harrogate area.

Main duties of the job
- Assist the caseload holder in assessing children/young people and their families' needs: supporting the implementation and evaluation of individualised packages of care.
- Work in partnership with the child, their families and colleagues from Health, NYC Children/Young People's services and Voluntary Organisations.
- Undertake home visits where appropriate to inform the assessment, care planning and evaluation process.
- To carry out an agreed plan of care, responding to any change in need, within given parameters and competencies.
- Follow care pathways and protocols, communicating with the case load holder; members of the team and other disciplines/agencies as appropriate.
- As delegated by the caseload holder provides an update on progress at Educational annual reviews for children on the caseload; amending health care plans as necessary.
- Support the delivery of competency based training for those children on the caseload, ensuring the safe delivery of nursing care to children and young people in any setting they choose to access.
- Contribute to the safeguarding of children through the awareness of safeguarding policies and procedures, updating knowledge appropriately and informing the caseload holder on any issues relating to the safeguarding of any child on the caseload.
