An exciting opportunity has arisen for a Community Anchor in our 0-25 Family Health Service in County Durham. this is a Countywide post. As a new non-traditional health role within the 0-25 Family Health Service contract, and unique to County Durham, the Community Anchor role has evolved over the past 2 years. The role:

  • Leads the 0-25 FHS Social Value agenda
  • Increases volunteering experiences
  • Engages with Area Action Partnerships (AAP) who are identifying community need and supporting funding streams to provide interventions.
  • Builds community capacity
  • Profiles communities to support service planning at place
  • Ensures the voice and needs of children, young people and families, and communities are included in design of activities and services
  • Embeds the County Durham Approach to Wellbeing

Main duties of the job

As part of the management team, the post holder will promote integrated and collaborative working and relationships with key partners to develop place-based care, ensure the best use of resources, and address health inequalities as is a key priority within this role.

Excellent people management skills, engagement skills, relationship building skills and resilience are required for this role. In addition, change management, project management, mobilisation of new activities or services, and a strong knowledge of governance are requirements.

The post holder will support the clinical and management team in developing and driving a culture of continuous quality improvement as well as ensuring teams are delivering high quality services efficiently.

Areas of specific Responsibility include:

Lead the Youth Worker Team to enable Children, Young People and Families, and communities to identify shared priorities and support the development of locally led initiatives to build stronger, safer and resilient communities empowered to promote protective factors to physical and mental health and emotional wellbeing.

Lead on the 0-25 Family Health Service integrated approach with the Early Help System in county Durham
