Job summary

This is a development role with the Apprentice working towards a HCPC registered, Graduate Physiotherapy role

The Apprentice will undertake a period of 3 years training, under the direction and supervision of a member of the team.

They will undertake: BSc (Hons) apprenticeship at Huddersfield University and be expected to complete all work based and academic elements to progress through the programme.

Once the Apprentice has:

Successfully completed the Apprenticeship Programme (including the End Point Assessment (EPA)

Achieved all work based competencies, including professional and performance measures

Met all the academic requirements

Has confirmation of the professional award/registration

They will be offered a substantive Graduate Physiotherapy, Band 5 post.

To work alongside qualified and Multi-Disciplinary Assistants (MDAs), single handed, or as part of a team, liaising with appropriate members of the community therapy team in relation to service delivery and patient care, using standard operating procedures and good practice guidelines and understands treatment outcome.

During the apprenticeship the candidate will experience the following areas:

  • Admission avoidance/Crisis response
  • Emergency department
  • Inpatient wards
  • Virtual Ward
  • Community bed based rehabilitation
  • Community based rehabilitation

Main duties of the job

  • To provide Physiotherapy assessment and interventions to a wide range of patients as required (Frail patients being the majority but not exclusive).
  • To work as a practitioner managing a defined clinical caseload of patients, being professionally and legally responsible and accountable for all aspects of own work
  • To work with patients, relatives and carers to identify Physiotherapy goals as part of an overall care plan
  • To take a responsible place in the multi-disciplinary team promoting the role of Physiotherapy to service users, other professionals and the general public
  • Responsible for carrying out home risk assessment visits to facilitate safe discharge home from hospital and supporting people to remain within their own homes, using specialist knowledge and skills
  • To take responsibility for delivering home assessment visits identifying and assessing patients' complex needs and risks during the visit with patients, carers and other health, social care and voluntary representatives present
  • Responsible for recommending, advising, negotiating, coordinating and implementing the complex interventions and service needs, of patients, necessary before returning home or to sustain at home
  • To take responsibility for health & safety and welfare of the patient at home including consideration of infection control and management of medical needs when appropriate and escalating to the multidisciplinary team if required
