Consultant for Virtual Ward and Acute Frailty Team.

10 PA with on call.

Harrogate and District NHS Foundation Trust (HDFT)are looking to appoint to a new consultant post to join our existing consultants who make up the acute frailty team. The post-holder would primarily be working within our new virtual frailty ward, with some sessions dedicated to working on the acute frailty unit within the hospital.The post will also have acommitment to out of hours working. The post is for 10 PA, which includes a 1 in 10-weekend working for the acute frailty team. Candidates wishing to work less than 10 PA are encouraged to apply as both full time and part time hours will be considered.

Main duties of the job

The development of frailty services is a priority for the Trust, both in theacute siteand incommunityservices.HDFT has a well-established, multidisiplinary 'Acute Response and Rehabilitation in the Community and Hospital' (ARCH) team which has been very successful at enabling our 'home first' ethos, and admission avoidance schemes. In line with national objectives and funding from NHSE, we are expanding this service further to incorporate 'Urgent Community Response' and meet the national specification for virtual frailty wards. The successful candidate would be an integral part of helping to develop the virtual frailty ward, allowing us to manage more complex and medically unwell patients in their own homes. This is an exciting opportunity to strengthen and expand pre-existing relationships with the community and primary care teams. In Harrogate we are lucky to have health and social care partners working together to improve how community health and social care is provided for adults in our locality via HARA (Harrogate and Rural Alliance) which brings together our commissioners, physical and mental health providers, primary care colleagues, and the County Council.

About us

You will be joining a dynamic and enthusiastic team of 12 Consultant geriatricians withcomplementary skills, and with special interests in acute care, community rehabilitation,movement disorders, stroke medicine, orthogeriatrics,continence, dementia,falls and polypharmacy. We are also supported by three specialist advanced clinical practitioners and 4 registrars, as well as a diverse and highly valuedmultidisciplinary team, who share our passion for delivering high quality person-centred care. As a community and acute frailty geriatrician, you will also work closely with colleagues inacute and general medicine,andin theEmergency Department.
