Job summary
HDFT are seeking to recruit an Early Years Practitioner Sunderland 0-19 to be part of their innovative and dynamic 0-5 team dedicated to promoting the health and wellbeing of children and young people .
You will work as part of 0-5 team working towards improved public health outcomes for children and families in Sunderland delivering high quality universal and targeted health services to meet the key improvement priorities.
The role will involve working within both community settings and including family homes and providing duty triage and managing a chat line at times .
Excellent communication skills and a passion for public health are essential, alongside good organisational skills, flexibility to meet the needs of the service, and ability to travel independently to meet service delivery as required.

Key aspects of the role include: Supporting health promotion planning and delivery, Health Needs Assessment, Multi-agency working to support children and families . You will be supporting interventions with families including , play, weaning , healthy eating and supporting children's development to name a few key areas.
