Job summary

An exciting opportunity has arisen within the Referral to Treatment (RTT) Team at Harrogate & District NHS Foundation Trust as the team is expanding.

We are looking for a passionate and proactive individual to join our hard working team who are responsible for the validation of patient pathways within HDFT to support the delivery of the Referral to Treatment (RTT) standard. The post holder will directly contribute towards the Trusts elective performance by monitoring patient pathways, reviewing the timeline data for patients on RTT waiting lists and ensuring that it correctly reflects the patients current position on the pathway. You will also play a role in getting the patient through their elective pathway with minimal delays.

The post holder is responsible for effectively co-ordinating the tracking of the administrative pathway using the Trusts new and recently implemented PTL (patient tracking list) software called LUNA Pathfinder, ensuring that all steps on the patient pathway are booked and recorded appropriately, in a timely manner. The post holder will use their own judgement and analytical skills to ensure patients pathways & 'clocks' are stopped and started correctly to ensure patient waiting times are recorded correctly. The post holder will develop a thorough understanding of the national rules for RTT.

The post holder may be required to work both on site at HDFT and work at home. Working a Wednesday is essential to attend PTL validation meetings.

Main duties of the job

In this role you will be responsible for monitoring the PTL by investigating each individual patient pathway within specialty groups and use multiple Trust systems to identify and validate the RTT status of patients who are currently on pathways. You will also be responsible for completing a series of data quality reports to ensure the Trust maintains a high level of data quality.

You will assess, analyse and interpret information recorded on the patient's administration systems using your own judgement to ensure patient pathways have been recorded correctly and rectifying any data quality issues within the pathway.

As part of this role,a general understanding of the patient journey throughout the hospital is required as you will be tracking patients throughout their journey from referral to treatment ensuring their appointments, diagnostic tests and any other activity is booked in a timely manner to support the patient through their RTT pathway and assisting with the prevention of breaches where possible.

To support the above, you will develop relationships with a range of departments Trust wide, work to motivate staff to achieve objectives and prevent breaches by working together in order to efficiently progress patients on their pathways. This is an exciting opportunity as in this role there is opportunity to identify areas for service improvement to streamline all steps of the patient pathway and improve quality and timelines of patient care.
