Job summary
Family practitioners will work within the 0-19 Healthier Together Service and will contribute to the provision of continued high- quality services for children, young people and their families. Family practitioners work directly with parents, carers, children and young people, helping them to build life skills, develop healthy relationships which helps individuals to make the decisions which are right for themselves.
Family practitioners utilise a strength- based approach, focusing on personal, social, emotional, cultural and educational development of children and young people. There will be a strong focus on building community capacity and resilience within local area. Developing and leading on projects such as HENRY and leading on other activities will be a key aspect of the role, including application for grants or funding requirements.
Main duties of the job
- To guide and support children and young people up to age 19 to give them confidence to excel in life. Ensuring coordination of HENRY offer. Engaging with vulnerable groups taking into account young people with Special Education Needs and Disabilities, young people in the Youth Justice Service and young people from the Gypsy, Roma and Traveller community.
- Empower parents and young people with practical skills alongside life skills/coaching.
- Signpost families and young people to community resources and government interventions.
- Organise activities to support young people with personal and social development.
- Ability to assess community need and understand the community
- Develop projects and workshops that deal with issues e.g. bullying, crime and risk tasking behaviours
- Develop and lead on group work such as HENRY programme
- Make Every Contact Count.
- Apply for grants and funding for projects as required
- Take part in outreach activities, both independently and in partnership with other services.
- Develop and build strong community relationships with key stakeholders
- Make contributions to policy development.
