Job summary

The Corporate Nursing Team has an exciting opportunity for an experienced Nurse/AHP to join the team as Enhanced Care Lead at #teamHDFT

We are looking for passionate individuals who are able to demonstrate empathy, compassion, person-centred care and the ability to cope in challenging situations. Good leadership, communication and de-escalation skills are essential. Knowledge, experience and awareness of the needs experienced by people living with cognitive impairment and/or delirium and skills to initiate therapeutic interventions are required.

Main duties of the job

  • The post involves reviewing and developing the enhanced care processes to standardise and ensure the safe and effective delivery of care to patients with enhanced care needs.
  • Through collaborative working with clinical areas daily, you will be contributing towards the assessment and review of patients requiring a higher enhanced level of care.
  • You will provide increased visibility to maintain patient safety and promote a positive patient experience through the supported delivery of individualised care and therapeutic interventions.
  • The post holder will be required to lead and support with Quality Improvement projects across the trust, to demonstrate the delivery of up to date practices, policies and protocols within the clinical environment in line with national and local guidelines.
  • The role involves the active organisation and delivery of training Trust wide in line with national guidance in the Dementia Strategy, the Butterfly scheme and John's Campaign
  • The role will play an active part, in partnership with others, to influence change and innovation in caring for people with dementia /and or delirium. There is close working with the Lead Nurse for Frailty and Dementia Lead to ensure ongoing development of skills and knowledge through relevant training, conference, and collaborative meetings.
  • Working alongside relevant partners in the voluntary sector and local community are also encompassed within the service
