Job summary
HDFT are seeking to appoint a Care Coordinator to work as a valued member of the Active Against Cancer team, to help support the delivery of an innovative health and wellbeing exercise and health referral service. This service is at the leading edge of exercise provision in cancer care in the community and is therefore an exciting opportunity for the post-holder to be involved in a positive project, pushing the boundaries of health care delivery.
The primary role of the post holder will be to work in the capacity of a care coordinator, triaging referrals, and therefore having regular direct patient contact. The post holder will also provide administration support to the service leads. Additionally, the post holder will have a role in coordinating service volunteers, communications and events for the service. The post will be based at Harrogate Sports and Fitness Centre.
The post holder will have experience in an administrative role, direct patient contact and will be enthusiastic about the work undertaken by the service. The post holder will require good organisational and communication skills and may have prior experience of working first-hand with vulnerable patients.
Main duties of the job
To work in conjunction with the service manager to liaise with Macmillan Clinical Nurse Specialist teams, the Macmillan specialist palliative care team and other allied health professionals to maximise the health and wellbeing of patients through exercise, improving the quality and efficiency of health care delivery.
To be responsible for the reception duties of Active Against Cancer, including effective communication with patients and staff in person, over the phone and by email.

Active Against Cancer is a new and unique free health and wellbeing service for people living with cancer in Harrogate. We provide bespoke, individualised activity programmes, free to all cancer patients, no matter their diagnosis or current fitness level. We have a dedicated team of highly specialised and friendly trainers. We are a busy service and the post holder will be expected to work on his/her own initiative and as a part of a team. .
