Job summary

We have an exciting opportunity for a motivated, enthusiastic and responsible Health Visitor SCPHN to work within our Growing Healthy Health Visiting Team in the Richmondshire team in North Yorkshire.

Main duties of the job

The Health Visiting role is to deliver the Healthy Child Programme as part of team, planning, delivering and evaluating an efficient and cost effective service for an identified population, you will be delivering the National and Local Public Health agendas in response to identified health and social need working with partner agencies in a strategic, creative and holistic way, ensuring the best outcomes for children, young people and families.

Health Visitors plan, organise and deliver programmes of care in conjunction with the family and the multi-disciplinary team and undertake continuous on-going assessment of children, young people and families. They provide an evidenced based service to children, people, families and communities in accordance with National and Local Policy initiatives.
