Tissue Viability Clinical Nurse Specialist

Job summary

We are seeking a dedicated and enthusiastic Band 6 Tissue Viability Clinical Nurse Specialist to join our team. You will be responsible for providing advice, teaching, management and care to patients with pressure ulcers and other complex wounds. Pressure ulcer prevention is a key part of this role.

You will be a key clinical leader, supporting the development and implementation of best practices, enhancing quality of care and empowering both patients and healthcare professionals with knowledge and expertise in Tissue Viability.

The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy. Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required.

Main duties of the job

Responsibilities for patient and client care

  • Assess, plan, implement and evaluate care and treatment using specialist clinical knowledge and skills, advising patients and relatives as appropriate
  • Be a non-medical prescriber - determined by the needs of the service
  • Organise own clinical workloadFreedom to act
  • Refer patients as appropriate to other members of the multi-disciplinary team
  • Make clinical assessments and treatment decisions and implement plans

Communication

  • Maintain accurate clinical and other records using the Trust's documentation
  • Support patients and their families communicating sensitive condition related information

Education and Training

  • Provide training and education of learners and other junior staff as part of the Tissue Viability education strategy
  • Actively participate in the development of own personal development plan and performance review
  • Provide education and training to patients and their carers, and promote a model of self-care where appropriate
  • Keep up to date with developments and advances within the specialty

Leadership and management

  • Investigate and respond to accidents, complaints, untoward incidents and other significant events
  • Under the direction of Lead Nurse participates in or leads the development of specific elements of nursing practice, reflecting the Chief Nurse Team and the Trust's Nursing and Midwifery strategy
  • Participate in the setting, monitoring and improvement of standards and the quality of patient care
