A dynamic team of heart failure specialist nurses are exploring different ways of working to deliver evidence-based practice to patients living with heart failure.

The hours advertised are up to 28 hours per week for a fixed term. During this fixed term period we will be working to secure permanent funding.
The successful candidate will have a minimum of 4 years' post registration experience, preferably with suitable heart failure nursing experience or a minimum of 2 years' experience of working in a cardiology setting. We have advertised the post as either a band 6 or 7 role, depending on experience. Support will be given to work towards heart failure specialist nurse competency framework set by the British Society of Heart Failure.
The post may become Permanent at the end of this Fixed Term/Secondment.

Main duties of the job
The post holder will be part of a team of experienced heart failure specialist nursesand be responsible for providing high quality expert care to patients with heart failurein York hospital and the Vale of York community setting.
The heart failure specialist nurse team deliver an out of hospital rapid access heart failure clinic based at a local health centre in central York,community nurse clinics in a variety of community setting across our Vale of York location and can support visiting patients in their ownhomes. The team are also building a heart failure in-reach model which focuses on admission avoidance and early step down to community delivered care.
