We are looking for a dynamic, dedicated and enthusiastic individual with the ability to enable and empower their team. Strong leadership and performance management is an integral of this role, with patient safety and patient experience underpinning all aspects of the role.

The primary aim of Transitional Care is to prevent avoidable term admissions to the neonatal unit, minimising separation of mother and baby. The post holder will be expected to implement a robust pathway of care to support Transitional Care on site and neonatal outreach service for York and Scarborough Hospitals.

Main duties of the job

Post holder will be responsible for implementing Transitional Care and expanding on the current Neonatal Outreach offering cross-site.

The role's key focus is to facilitate the implementation and evaluation of care pathways within Neonatal Transitional Care (NTC) and Neonatal Outreach. This focuses on keeping mothers and babies together and preventing avoidable admissions to the neonatal unit.

T The post holder will need to ensure that they keep up to date with Neonatal Transitional Care and Neonatal Outreach developments in line with national guidance and requirements.
