Job summary
We are currently seeking a confident individual to join our existing Palliative Care Hospital Team based at York hospital.
This post offers an exciting opportunity to join an existing service that is looking to develop services.
The role will involve close working with members of the multi-disciplinary team, to coordinate the needs of patients experiencing a difficult and often demanding health concern. The post holder would act as the point of contact for the nursing service. They would be responsible for triaging calls and proactively identify and take action, to ensure that patients experience a seamless pathway.
Main duties of the job
Collaborate and communicate with the wider healthcare team, acting as a resource to ensure the provision of appropriate services to maximise the health and quality of life of the patient, improving the quality and efficiency of healthcare delivery. The post holder will be responsible for the coordination of the administrative function within the complexity of specialist palliative care.
The post holder will be expected to manage and prioritise the running of the service, and work flexibly to the needs of the service, which can often be demanding and unexpected.
The post holder will be responsible for maintaining accurate patient records, and liaise across organisational boundaries to assist the clinical nurse specialist to develop the nursing service and provide the highest quality patient care in accordance with the National Ambitions for Palliative and End of Life Care.

- Educated to GCSE/O' level standard in core subjects or evidence of working at that level
- Computer literate with knowledge of IT systems such as Word and Excel
- Willingness to undertake statutory and mandatory training and to undergo further training as appropriate
- HNC or NVQ Level 3 or equivalent evidence of working at that level
- Evidence of continued professional development.

- Administration qualification
- Knowledge of databases and spreadsheets
- Understanding of training needs in relation to post

- Experience within the NHS or similar environment (e.g. Dept of Health or other public service)
