Job summary
A fantastic opportunity has arisen for an enthusiastic, motivated and experienced clinician to join our Community team in the role of Advanced Clinical Practitioner (ACP) within Integrated Care, Community Services at York and Scarborough Teaching Hospitals NHS Foundation Trust. We are seeking candidates with a passion in providing acute adult care, to work within our Virtual Frailty Ward and Urgent Crisis Response, community services.
The post holder will work within locally and nationally agreed protocols and guidelines in order to provide assessment, investigation, diagnosis, evaluation and treatment planning, supporting admission avoidance and facilitation of early hospital discharge for patients referred to our Virtual Frailty Ward and Urgent Community Response services, supporting patients within their own home environment.
Main duties of the job
The ACP will provide a high level of expert clinical care and ensure that evidence- based practice is inherent in all aspects of care and treatment, as well as promoting clinical leadership and standard setting for the service. You will work as an integral part of the multidisciplinary team, working closely with acute and community physicians, taking referrals from Primary and Secondary Care across York, City and Vale localities.
The role offers an exciting opportunity to be part of a new and developing service model, providing acute care within the community. The post holder will work collaboratively with the Consultant Geriatrician, Clinical Director, and GP's, taking part in a rota that covers a seven-day service during the hours of 8am and 8pm.
The successful candidate will be supported in learning and development opportunities, as provided to Junior Doctors within the Trust and run by the Medical Education Department and will be supported in clinical supervision by the Consultant team.

- Registered Nurse or Allied Health Professional (with current valid registration)
- Master's degree in advanced clinical Practice including advanced clinical examination skills and non-medical prescribing qualification
- Willing to undertake further training as necessary to undertake role
Experience and Knowledge Required

- 6 years post base profession qualified experience which should include in depth knowledge and experience in the specialist area - care of elderly/community services
- A minimum of four years as qualified ACP with relevant experience
- Evidence of management and leadership
- Experience of role specific advanced clinical skills e.g., interpretation of ECGs
