Job summary

As an experienced registered nurse the jobholder will support the co-ordination of in-patient diabetes care over 7 days, ensuring effective communication is maintained at all times, providing support to qualified and unqualified nurses within the ward/department.

You will play a key role in the assessment of the needs of inpatients with diabetes across the Trust, identification of potential and actual problems, and implementation of an effective care plan.

If required the post holder will offer advice and support to Health Care Professionals in the community to facilitate a smooth discharge.

Main duties of the job

The jobholder will act as a role model, promoting quality evidence-based practice, supporting nursing and medical teams across the Trust and ensuring effective management of the in-patient diabetes service in the absence of the lead in-patient specialist nurse.

The post holder will be expected to support the lead in-patient diabetes specialist nurse in the undertaking of high quality audit and research designed to evaluate and improve the service.

The post holder will provide direct care to patients with diabetes in hospital according to the needs of the patients and their families. They will work autonomously as well as within the multi-disciplinary team in order to provide expertise and education to all health care professionals and carers with whom the patient with diabetes may come into contact during an admission to hospital. They will be expected to support healthcare professionals in the delivery of excellent in-patient care, improving safety, reducing risk and promoting patient self-management where appropriate.
