Job summary
The ward consists of 33 beds and is led by an approachable Ward Manager supported by 6 excellent Team Leaders. Shand is a stroke ward, with acute and stroke rehabilitation patients and complex discharges. The care delivered is of a very high standard and the whole team works closely to ensure all patient experience is positive. The ward work towards the standards, patient rehabilitation, safety, person-centred care, elimination, pain management and End of life care.
Although we have a very strong learning and development function, we appreciate you will need to have acquired some skills and experience from your career you will require for your time with us to be a real success.
Main duties of the job
The post holder will be responsible for supporting the sister/charge nurse on the ward in the management and delivery of patient centred care to patients admitted to the unit. The role incorporates assessing needs, developing, implementing and evaluating programmes of care, supporting and facilitating appropriate rehabilitation opportunities, safe discharges and promoting patient independence.
The role will:
o The role will support the senior sister/charge nurse to ensure that the unit to maintaining a safe and effective service at all times. 1 Communicate and work well within a multidisciplinary team.o Deliver person centre care to meet the assessed needs of patients and significant others.o Willingness to expand your stroke knowledge and best practice initiatives.
