Job summary
To provide administrative support to the MDT Team, covering MDT meetings, tracking patients through their pathways and ensuring that robust data collection is in place.
Main duties of the job
Patient Navigating:
Prospective and proactive management of the inter-trust cancer pathway, monitoring, reporting and tracking cancer patients, ensuring that the patient's journey is processed in a timely and efficient manner, in line with the nationally agreed pathway and with the Cancer Waiting Times Targets and guidelines.
Resolving delays in the pathway at the earliest opportunity and where this is not possible escalating issues in accordance with Trust and regional procedures to prevent treatment occurring beyond the target timescales.
S/LMDT:
To provide cover as and when required to support MDT meetings.
Co-ordinating and providing administrative cover support to the cancer S/LMDT(s) during periods of annual leave and sickness, ensuring that all relevant paperwork and clinical information is available including case notes, imaging and pathology as necessary. To attend S/MDT'S keeping comprehensive records of attendance, outcomes as discussed.
Ensuring the timely and efficient transfer of patients and information between trusts involved in the patient pathway, adhering to regional and nationally agreed communication pathways and protocols.
Data collection and analysis:
To support the accurate and timely collection of cancer waiting time (CWT) data in accordance with National Cancer Waiting Times Monitoring Dataset guidance and to report at weekly PTL meetings.
