Job summary

The Diabetes Specialist Nurse will be a highly specialised (advanced, expert,) autonomous practitioner managing a locality area within the geographical catchment area of Bedford Hospital. They will act as a specialist resource working alongside and supporting community staff to meet the needs of people with diabetes in all community settings within the allocated locality including GP surgeries, Nursing Homes, hard to reach groups etc. He/she will play a lead role in helping practices implement evidence based practice and will contribute to the provision of a range of services to those with diabetes, participating in the management and delivery of treatment plans as necessary.

The post holder, as a member of the Integrated Community Diabetes Service (ICDS), will deliver training and education packages to both patients and Health Care Professionals across primary and secondary care

Main duties of the job

  • To act as a resource and role model in expert practice facilitating and supporting the delivery of direct specialist diabetes patient care for all members of the Primary health care team, nursing and residential homes. Voluntary organisations and other agencies.
  • To provide clinical supervision for other Health Care Professionals (GP's, Practices nurses, District Nurses, Nursing/ learning disability homes) in designated area of community.
  • Be the responsible lead in Diabetes Service development within the given locality, this may include services for the elderly, housebound etc. To evaluate service delivery; identify areas for improvement and plan the future development.
  • To provide Specialist Diabetes Triage /Assessment clinics within the community setting

  • Registered Nurse with current NMC registration
  • Educated to degree level or equivalent
  • Teaching and assessing qualification or ENB 998 or equivalent

Desirable

  • Post registration qualification in Diabetes
  • Evidence of Masters level studies

  • Experience in working in the field of Diabetes
