Job summary
We are looking for an experienced professional with good communication and collaborative skills. You would need to be a committed individual able to work within the multidisciplinary team. Archer unit is a 20 bedded Physical Rehabilitation Unit. You will be providing care for mainly elderly patients with wide range of medical / surgical conditions.
Due to the nature of our patient's conditions, they all require complex discharge planning. There will be regular liaison with therapists, social workers and the palliative care team when necessary providing advocacy for vulnerable patients with dementia and other cognitive impairments. The role incorporates supporting and facilitating appropriate rehabilitation opportunities, safe discharges and promoting patient independence. You will be required to rotate shifts across days, weekends and nights.
Although we have a very strong learning and development function, we appreciate you will need to have acquired some skills and experience from your career so far.
Main duties of the job
The post holder will be responsible for supporting the unit in the management and delivery of individualised care to patients admitted to the unit. supporting and facilitating appropriate rehabilitation opportunities, safe discharges and promoting patient independence.
The role will:
o The role will support the senior sister/charge nurse to ensure that the unit to maintaining a safe and effective service at all times. Communicate and work well within a multidisciplinary team.
o Deliver person centre care to meet the assessed needs of patients and significant others.o Willingness to expand your elderly and frail knowledge and best practice initiatives.
