Job summary

Shand Stroke Unit is a 32 bedded ward - 20 beds for stroke and neurological recovery patients and 12 beds for acute medical patients . You will coordinate each shift ensuring safe efficient and effective care is delivered 24hrs a day.

The team you will join is a stroke and medical based team consisting of two consultants. Shand Ward cares for every medical illness with a diverse type of clientele. We strive to deliver an exceedingly high standard of individualised nursing care over every 24hr period.

Main duties of the job

1. Act as a professional role model to junior staff and demonstrate leadership qualities.2. Provide expert clinical knowledge and skills within the ward/department.3. Work with the ward manager and nursing team to develop and promote best nursing practice.4. Work as part of the ward team to assess, plan, implement and evaluate programmes of care in line with best practice.5. Ensure that self and other professionals adhere to infection control best practice and policies in all aspects of clinical work.6. Seek expert advice and support from specialist nurses when planning and delivering care to patients.7. Maintain accurate and clear documentation of all nursing care and communication with the multidisciplinary team.8. Communicate with patients and relatives in a sensitive manner, providing information regarding care and progress as required.9. Support bereaved and distressed relatives when required, referring to senior staff or bereavement service as required.10. Liaise with the multidisciplinary team to ensure clinical care is optimised from admission to discharge.11. Facilitate discharge of patients to enable safe, appropriate and timely discharge with suitable ongoing arrangements for care needs. Communicate effectively with the matron, bed management team and discharge coordinators to optimise use of inpatient beds.12. Actively assist the ward manager in implementing change and service developments.
