As a Band 6 Diabetes Nurse Specialist, the successful applicant will share the ambition, vision and values of the department and the wider Trust, supporting a group of Diabetes Specialist Nurses and the wider multidisciplinary team including Dietitians and Diabetes Assistant Practitioners.
Main duties of the job
Bedfordshire NHS Hospitals Foundation Trust offers a very exciting opportunity to the successful candidate to be part of a dynamic Diabetes Nursing Team; based at the Luton and Dunstable Hospital.
The successful candidate will be responsible for ensuring a safe, seamless and exemplary care at the point of need, within the inpatient and outpatient diabetes services including In-reach, pregnancy and mental health; whilst engaging with integrated and community services.
The post holder will be accountable for the delivery of high quality, patient centred care within an agreed financial envelope.

The post holder will work as a member of the Diabetes Specialist Team contributing to the provision of a range of services to those people with diabetes
either under secondary care or admitted to Luton and Dunstable Hospital as an inpatient. The post holder, in conjunction with the multidisciplinary team, will develop the diabetes service across secondary care.
The role will be mainly within the inpatient setting but there will also be elements of outpatient work.
OBJECTIVES AND PURPOSE OF THE POST:
Key Tasks and Responsibilities:
- a) Maintain and supervise group and individual diabetes related teaching held for:
Patients, relatives and carers/supporters
Health Care Professionals.
- a) Receive direct patients referrals and triage from:
Diabetes Team
Primary Care
Other specialities
- Assess the individual needs of patients and provide the relevant information and care required in the management of diabetes.
- Adjust insulin/oral hypoglycaemic agents (OHA) in accordance with Trust Protocol, thus enabling the post holder to:
- Initiate insulin/oral therapy in the inpatient and outpatient hospital settings.
- Stabilise poorly managed diabetes.
- Advise and adjust insulin OHA doses during illness and other special situations as identified in Protocol.
- Counsel patients and relatives in social, psychological and emotional problems related (or not) to their condition.
- In conjunction with the MDT, be responsible for own inpatient case load and have full responsibility and accountability for designated wards on a daily basis.
- Be responsible in leading in a specialised area of diabetes with a plan for developing the service further.
- Plan, deliver and evaluate education programmes for other Health Care Professionals and patients in designated area of diabetes care.
- Liaise with, update knowledge and advise medical/nursing staff regarding the care and treatment of diabetes.
- Liaise with community nursing, GPs and medical staff to ensure a comprehensive diabetes service is provided across primary/secondary care.
- Liaise with the multidisciplinary team involved in diabetes care.
- Assist and participate with the Lead/other Diabetes Nurse Specialist in formulating policies relating to diabetes care.
- Participate in research/surveillance in all aspects of diabetes and provide ongoing information for other members of the diabetes care team.
- Provide clinical supervision to other team members and health care professionals.
- Provide a positive public relations image by dealing with queries direct from members of the public.
- Report all accidents and complaints in accordance with agreed policies and assist in investigations of complaints and accidents as required.
- Uphold the principles of the NMCs Code of professional conduct.
