Job summary
We are seeking a motivated and organised individual to join our Upper GI team as a Band 4 Cancer Care Co-Ordinator.
The primary aim of this role is to support the Clinical Nurse Specialist (CNS) Teams. The post holder will play a key role in the organisation of services, both by supporting the CNS's administratively and by providing a single point of access for patients with the aim of optimising outpatient care. They will support the CNS's to deliver quality cancer care to and will also help to facilitate Holistic Needs Assessment (HNA).
The post presents an opportunity to join an established Upper GI Cancer Nurse Specialist Team as a Band 4 working 37.5 hours (1.0 WTE).
Excellent communication, organisational and prioritisation skills are essential.
Main duties of the job
We are seeking an organised and proactive individual to provide vital administrative support to the Clinical Nurse Specialists (CNSs). The post holder will help coordinate patient care, manage appointment bookings and outcomes, maintain accurate patient databases, and ensure efficient operation of the service.
Working closely with the UGI CNS team, the role involves supporting patient pathways, facilitating smooth handovers between teams, and communicating effectively with patients, carers, and multidisciplinary staff. The post holder will act as a single point of access for the service, triaging telephone and email enquiries, and ensuring timely responses, particularly for those with urgent or specialist needs.
Additional responsibilities include providing information and support about cancer services using Trust-approved materials, signposting to organisations such as Maggie's and Macmillan, and recognising situations requiring urgent clinical response. The successful candidate will ensure accurate documentation, advocate for patients where needed, and assist with the rollout of Holistic Needs Assessments (HNAs) and electronic patient-reported outcome measures (ePROMs).

The UGI team is a friendly, approachable and dynamic team of health professionals that consists of Clinical Nurse Specialists (CNS's) and a wider multi-disciplinary team with a shared goal to provide effective, timely and compassionate care for patients with UGI cancer. The post holder will work directly under the supervision of the Clinical Nurse Specialist and will also work alongside whole team to support the service users.

DUTIES AND RESPONSIBILITIES
- Provide administrative support to the CNSs.
- Book and outcome telephone clinic appointments
- Maintain the patient Database and ensure all of the information is up to date and accurate
- Order supplies and patient information as requested by the CNSs.
- Assist the CNSs with the management of the patient pathway ie ensuring that appointments have been requested and booked
- Organise and prioritise the designated workload to meet the needs of the service. Assist the CNS with handover of patient care to other teams in order to facilitate safe and effective transition of care. Communicate effectively with a range of patients and health and social disciplines. This may include a variety of methods, such as electronic or in person.
- Provide a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs.
- Triage telephone and email enquiries, using a risk assessment framework.
- Ensure patients/carers know how to contact the service when their condition / needs have changed, including out of hours.
- Provide general information and support about cancer services, using Trust approved literature. Sign-post patients and relatives/carers to appropriate services such as Maggies and Macmillan for them to speak to professionals who are able to discuss issues knowledgeably and impartially.
- Demonstrate the ability to recognise situations that require rapid response from a health care professional (CNS or doctor) and respond appropriately. Evaluate outcomes of care delivery with the registered practitioner.
- Act as an advocate and facilitator for patients; linking with CNSs to resolve any issues.
- Ensure accurate documentation of patient care in accordance with hospital policies and standards.
