Job summary

We have a new exciting opportunity for a Cancer Care Co-ordinator's to join our Early Breast Team.

The successful candidate will be passionate about making a difference to patients diagnosed with early breast cancer. If successful you will be offered an opportunity to work within this oncology speciality across varied clinics and patients.

Excellent communication, organisational and prioritisation skills are essential. Previous experience of working within Breast Oncology would be desirable.

Main duties of the job

The post holder will work alongside the Early Breast Team Clinical Nurse Specialists (CNS) /Advanced Clinical Practitioner (ACP) teams, their responsibilities will include:

  • Assisting the CNS /ACP team with the facilitation and implementation of patient's treatment plans within the disease group.
  • Collaborating with various teams both internal and external to the trust.
  • Responsible for triaging phone calls and emails to the CNS /ACP service.
  • Providing support with administrative services including referrals to varied multiple disciplinary teams.
  • Attend relevant clinics with the CNS/ACP team and interact with patient and relatives.
  • Willing to undertake relevant training days within the disease group and cancer pathway.
  • Willing to attend relevant MDT sessions if needed.
  • Signposting patients to relevant services.

About us

You will be part of a core team of CNS and ACPs within a wider team of consultants, registrars, breast care nurses, breast surgeons, and research staff.

The team provides a comprehensive service to patients with early breast cancer. These patients are receiving treatment with curative intent undergoing a combination of treatment with HER2 targeted therapies and/or ovarian function suppression with medication.

Job responsibilities

DUTIES AND RESPONSIBILITIES

  • Provide administrative support to the CNSs and ACPs.
  • Maintain the patient databases and ensure the information is up to date and accurate.
  • Assist the CNSs and ACPSs with the management of the patient pathway.
  • Organise and prioritise the designated workload to meet the needs of the service.
  • Assist the CNSs and ACPs with delivery of patient care with other teams in order to facilitate safe and effective transition of care.
  • Communicate effectively with a range of patients and health and social disciplines.
  • Support service improvements and audits where necessary.
  • Order supplies and patient information as requested by the team.
  • Provide a single point of access for patient care.
  • Triage telephone and email enquiries, using a risk assessment framework.
  • Ensure patients/carers know how to contact the service when their condition / needs have changed, including out of hours.
  • Provide general information and support about cancer services, using Trust approved literature.
  • Sign-post patients and relatives/carers to appropriate supportive services such as Maggies, Breast Cancer now and Macmillan.
  • Demonstrate the ability to recognise situations that require rapid response from a health care professional (CNS, ACP or Doctor) and respond appropriately.
  • Act as an advocate and facilitator for patients, linking with the team to resolve any issues.
  • Ensure accurate documentation of patient care in accordance with hospital policies and standards.
  • Assist the team with electronic patient outcomes (eproms) within the disease group

Professional/Clinical Leadership:

  • To act as a role model to colleagues, and be an effective resource to patients, carers and colleagues.
  • Support clinical developments and projects to promote excellence in patient care. Assist in evaluation and audit of such developments
  • Attend and participate CNS Away Days and Strategy Meetings
  • Recognise personal limitations and work within parameters of job description, or as guided by CNS. Seek help appropriately.

Education and Development

  • To participate in annual appraisal and accept personal responsibility for growth and development within the role.

  • Attend educational meetings and conferences, appropriate to the role, to keep up-dated with current best practice.
