An exciting opportunity has arisen for a Diabetes care co-ordinator (band 4) to join the diabetes service at the Christie Hospital to support the management of patients with diabetes and those at risk of developing diabetes. We are looking for a highly motivated, innovative and compassionate individual. You need to be committed to improving diabetes service delivery.

We are seeking a compassionate and experienced Diabetes Care Coordinator to join our multidisciplinary team at a leading oncology hospital. In this vital role, you will support cancer patients who are also managing diabetes, ensuring they receive comprehensive, personalized care that addresses both conditions. We would also welcome applications from individuals with other NHS experience who have an interest in both diabetes care and in working with people with oncological conditions.

Main duties of the job

As a Diabetes Care Coordinator, you will work closely with oncologists, endocrinologists, nurses, dietitians, and other healthcare professionals to develop and implement individualized diabetes management plans. Your focus will be on patient education, medication management, and lifestyle support, helping patients maintain optimal glycemic control throughout their cancer treatment journey. The successful candidate will act as a vital link between oncology and diabetes services, providing education, clinical support, and care coordination. You will empower patients and their families with the knowledge and tools to manage diabetes effectively during cancer treatment, including understanding the impact of chemotherapy, steroids, and other medications on blood glucose levels.

You will also play a key role in triaging patient concerns, coordinating appointments and follow-ups, and supporting the diabetes specialist nursing team with administrative and clinical tasks.

You must be self-motivating, organised, computer literate and able to demonstrate excellent interpersonal skills with a commitment to advancing diabetes care through collaborative working with oncology teams and personal development.

DUTIES AND RESPONSIBILITIES

  • Act as a liaison between oncology and diabetes care teams to ensure continuity and consistency in treatment.
  • Communicating effectively (face-to-face, telephone, remote/virtual) with a range of individuals on various levels, e.g. patients, relatives, carers, other health / social care professionals and community workers as required.
  • Providing general information to individuals in a range of different formats and support to patients in a personalised manner to support diverse range of people.
  • Provide advocacy for patients, helping individuals navigate health and social care, staying well informed, empowering them to make the best choices for their needs.
  • Support the diabetes specialist nursing team with the administration of on-going patient care including referrals to different hospital, primary care, community and specialist teams.
  • Triage telephone and email enquiries, initiating an appropriate response according to risk assessment protocols and individual pathways (escalating urgent and referring complex decisions to the nursing/senior team for assessment and review).
  • Make pre-planned telephone calls to patients to assess on-going needs and to enable a proactive prevention approach taking into consideration the views of patients and expertise, arranging other methods of assessment should this be required.
  • Co-ordinate handover of patients with other teams to facilitate safe, effective seamless transition of care between services.
  • Recognising professional boundaries and role limitations, make appropriate decisions to seek help and report concerns to colleagues when an individuals choices place them at risk.
  • Assist with the planning and delivery of Health and Wellbeing events/offer and/or support groups or where appropriate signposting patients to other offerings.
  • Promote healthy lifestyle choices, provide guidance on diet, exercise, glucose monitoring, and medication adherence.
  • Co-ordinate and organise appointments, as needed, including communicating concerns with clinical team where fast tracking/rapid re-entry of individuals back into the system may be required.
  • Ensure patients and carers know how to contact the service when conditions or needs change, including out of hours.
  • Support diabetes specialist nurse to coach patients/carers to understand side-effects of treatment. Support the delivery of patient and carer training and education.
  • Demonstrate and awareness of the limits of own practice and knowledge and when to seek support/advice.
  • To check stock levels of equipment required by the diabetes team and order as appropriately.
  • Provide administrative support to the diabetes nursing team, including coordinating appointments, managing referrals and maintaining accurate patient records.

EDUCATION, TRAINING AND AUDIT

The post holder will:

  • Support the delivery of patient and carer training and education within scope of practice. Reinforce patient education provided by nurses or diabetes specialists, such as foot care, signs of hypo/hyperglycemia, and medication adherence.
  • Support and contribute to audit processes, governance, research, clinical research trials, benchmarking activities and service development, including using findings and recommendations to change practice/systems as appropriate.
  • Develop, manage and maintain databases to record activity; monitor the processing of this data.
  • Understand the ethical and legal issues around data collection and information handling, including confidentiality, consent, data protection and storage
  • Evaluate outcomes of care delivery with the registered practitioner.
  • Analyse and interpret information and be able to present it different ways so it can be easily understood at a wide range of levels.
  • Take responsibility for personal development and education by demonstrating self-directed learning, actively seeking professional development opportunities to enhance practice, knowledge and role progression.
