Job summary
We have an exciting opportunity for a Cancer Care Co-ordinator to join our clinical and medical oncology team.The successful candidate will be passionate about making a difference to patients diagnosed with cancer supporting the gynaecology division.
Main duties of the job
The post holder will work alongside the disease specific CNS /ACP teams, their responsibilities will include
- Assisting the CNS /ACP team with the facilitation and implementation of Holistic Needs assessment within the disease group.
- Responsible for triaging phone calls and emails to the CNS /ACP service.
- Providing support with administrative services including referrals to Macmillan and District Nurses.
- Attend relevant clinics with the CNS/ACP team and interact with patient and relatives.
- Willing to undertake relevant training days within the disease group and cancer pathway.
- Willing to attend relevant MDT sessions if needed.
- Signposting patients to relevant services.
